CONSULT INSTRUCTIONS FOR USE.
Unique Device Identifier (UDI); the Application Identifier (01) represents the
device identifier (DI) ("0697074222XXXX"), the Application Identifier (11) the
production date/date of manufacture ("YYMMDD", with "YY" the last two digits
of the production year, "MM" the production month and "DD" the production
day), the Application Identifier (21) the serial number of the device ("WWWWW-
WWWW").

Indication of conformity with the essential health and safety requirements set
out in European Directives 0120 and 1639 – Notified body identification (SGS).

DATE OF MANUFACTURE. This symbol will be accompanied by the date of
manufacture.
MANUFACTURER. This symbol will be accompanied by the name and the
address of the manufacturer.
AUTHORIZED REPRESENTATIVE IN THE EUROPEAN COMMUNITY. This
symbol will be accompanied by the name and the address of the authorized
representative in the European Community, adjacent to the symbol.
