INDICATIONS FOR USE / INTENDED USE
The intended use of this device is to immobilize the head during radiation therapy and to support and position during
an MRI.
CAUTION
Federal (United States) law restricts this device to sale by or on the order of a physician.
MRI SAFETY INFORMATION
Device is MR safe.

WARNING
Verify all angles of treatment, attenuation characteristics and WET values prior to treating patients.
l
Product shape may have slight variations; the same device should be used for a patient throughout setup
l
and treatment cycles.
Do not use if device appears damaged.
l
Ensure accessory device is properly located.
l
Ensure device is secure prior to use.
l
Verification image should be taken to confirm patient position prior to treatment.
l
Ensure device/patient does not move for duration of set-up and treatment.
l
Do not use excessive force on device. Injury or device damage may occur.
l
Device is for use with CIVCO Radiotherapy specified accessories only.
l
NOTE: If any serious incident occurs in relation to device, incident should be reported to manufacturer. If incident
occurred within the European Union, also report to the competent authority of the Member State in which
you are established.
USING HEADRESTS
1. Place headrest onto device.
NOTE: Ensure notches are placed into locating features.
2. Position patient on headrest.
